The Community Care Coalition (CCC) project model is the foundational community structure for supporting various possible types of programming to ensure sustained child well-being for the most vulnerable boys and girls at the local level. The CCC also provides resources and family strengthening for families as a whole.
The CCC project model, originally part of the African American Family Institute initiative, seeks to strengthen community-led care and support for children and families. CCCs are formed by joining key community stakeholders into a coalition to identify Social Determinants of Health-related issues within the community and to develop and prioritize action plans for responding to these issues. CCCs normally recruit and supervise Parent Leaders (PLs), who are trained by AAFI staff, to Host parent café’s and to provide essential forms of care and support families in need.
The initial Community Care Coalition was initiated within the faith community with three initial organizations, Stewart Memorial United Methodist Church, St. Timothy’s Episcopal Church, and New Heart Christian Center. The invitation is open to all who seek to help strengthen families and transform the community in which we live.
The CCC project model may work with community groups mobilized by African American Family Institute, as well as pre-existing groups that AAFI may work collaboratively with and through.
The CCC model may be adopted as a foundation for other types of programming options.
